School Outreach Service

MindCare Psychology Services acknowledges that schools play an important role in supporting the mental health needs of children, adolescents and their families. Committed to improving the wellbeing and mental health of students, MindCare is running an outreach psychology service to schools in the Metro Melbourne area. The outreach service links a psychologist with a school to offers their students the opportunity to receive counselling at school during school hours.

General Practitioners

A Mental Health Care Plan (MHCP) is required for clients to access Medicare Benefits.


Please provide a dated and signed MHCP referral letter to your client for them to bring to their first appointment or fax to (03) 9960 7501. For clients to be entitled to the Medicare rebates please insure the MHCP states how many session are approved. A review of the clients MHCP will be provided directly to the referring GP via email or fax from the clients psychologist.

Psychiatrists

We require a referral letter for individuals referred by a psychiatrist. For your convenience, a pre-formatted Referral Letter Template with our contact details is available below.

Other Health Professionals

A referral is not required for health professionals (other than a GP or psychiatrist) wishing to refer a child, adolescent or adult. Please contact us directly if you wish to discuss a referral further.
